Herbal and medicinal beers are brewed with botanicals — bittering, aromatic, and preservative herbs — in place of, or alongside, hops. The oldest name for this is gruit: the herb blend that flavoured European beer for centuries before hops took over.
What is gruit, and why brew without hops?
For most of brewing history, beer wasn’t bittered with hops at all. Brewers used gruit — a blend of herbs that supplied bitterness, aroma, and a measure of preservation. Hops only became dominant between roughly the 15th and 17th centuries, pushed along as much by taxes, trade, and religious politics as by taste. In the Middle Ages the right to sell gruit was often a controlled, taxed monopoly, so the herb blend was quietly at the centre of the medieval economy.
Brewing herbal beer today is partly historical re-creation and partly a flavour choice. Without hops’ pine-and-citrus signature you get room for something else entirely: the resinous bite of yarrow, the sweet eucalyptus lift of sweet gale, the honeyed floral note of heather. You can go fully hop-free for an authentic gruit, or keep a small hop backbone for stability and let herbs carry the character.
The classic gruit trio
The blend most associated with historic gruit centred on three herbs. Historians caution that recipes varied wildly by region and that yarrow was probably the real workhorse — but this trio is the traditional starting point.
A note on names: “wild rosemary” and “marsh rosemary” in a gruit context mean Rhododendron/Ledum, not the garden herb Rosmarinus. The garden one is fine and pleasant in beer; the marsh one is a genuine-caution botanical. Sweet gale (bog myrtle) is the safe, food-grade aromatic of the three and the one most modern gruit brewers actually reach for.
The three jobs an herb can do
Hops conveniently do everything at once — bitter, aromatic, and preservative. When you brew with herbs you assemble those roles yourself, usually from two or three different plants. Think of every herb as filling one primary slot.
Common brewing herbs and what they bring
Beyond the classic trio, plenty of accessible herbs make excellent beer. Read this as a signal list — sage tones are easy, food-safe, forgiving; terracotta tones need a light hand.

How to add herbs: boil, late, dry, or tea
When you add an herb matters as much as which herb it is. The rule mirrors hopping: heat extracts bitterness but destroys aroma, so bittering goes early and aroma goes late.
- 1Boil (bittering, 15–30 min) Add sturdy bittering herbs — yarrow, mugwort, horehound, roots — for the last 15–30 minutes of the boil. This extracts bitter compounds and sanitises the plant matter. Longer isn't better; most bitterness is out within 30 minutes.
- 2Late / whirlpool (aroma, last 0–10 min) Add aromatic herbs in the final 5 minutes or at flameout/whirlpool (below ~80°C). You keep the volatile oils that a hard boil would strip. This is where sweet gale, heather, citrus leaves, and juniper shine.
- 3Dry / secondary (pure aroma) Steep clean, dried herbs in the fermenter after primary — the herbal equivalent of dry-hopping. Maximum aroma, zero boil-off, but no sanitising heat, so use clean dried herb and keep contact to a few days.
- 4Tea / tincture (control & safety) Brew the herb as a strong tea or alcohol tincture separately, then blend measured amounts into the finished beer to taste. This is the safest way to dial in strong or cautionary herbs — you never over-commit a whole batch.
The tea method is the intermediate brewer’s secret weapon: brew the beer clean and bitter, then blend aromatic and cautious herbs to taste at packaging. You get precision instead of a whole-batch gamble.
Dosing: start low, scale up next time
Herbs have no IBU rating and vary hugely in potency batch to batch, so there’s no clean formula — only sensible starting points for a 20L (5 gal) batch. Treat these as first-batch floors, not targets.
Because you can always add more next time but can’t un-bitter a batch, undershoot on the first brew and take notes. If it lands mild, step up 25–50% next round. Keeping a small hop charge in the recipe also gives you a familiar reference point — you can size it with an IBU estimate and let the herbs fill in around it.
Balancing a beer without hops' preservative power
Hops don’t just bitter beer — their iso-alpha acids are strongly antimicrobial and are a big reason hopped beer keeps for months. Most brewing herbs, even mildly antiseptic ones, don’t come close. So an herb-forward beer needs its stability engineered in elsewhere.
Watch for
- No hops means no built-in antimicrobial shield — assume the beer is more perishable than a hopped one
- Don't age herb-only beers long; plan to drink them within a few weeks to a couple of months
- Under-pitched or warm ferments leave sugar and stress the beer — pitch healthy yeast and ferment fully
- Warm storage accelerates spoilage — keep herbal beer cold once packaged
The practical fixes are all in your control: brew and package cleanly, ferment out completely, bump the ABV a little (alcohol is itself a preservative), keep it cold, and drink it young. If you want the herb character and real shelf life, add a small hop charge — even 5–10 IBU of a neutral bittering hop gives meaningful protection while leaving the herbs firmly in front.
'Medicinal' is tradition, not medical advice
Medieval brewers reached for herbs partly for their reputed effects — bitter tonics, aromatic “heady” ales, gently sedative blends. That’s why these beers are called medicinal. It’s genuine historical and culinary context, but it is not a health claim, and nothing here is medical advice.
Most gruit herbs — yarrow, sweet gale, heather, juniper, garden rosemary, lemon balm — are used at modest culinary doses and are fine for the average adult. A short list of traditional botanicals genuinely warrants caution:
